▪️If you are under the age of 20, you must also have a parent or guardian's signature and seal.
▪️Initial fee
▪️ Proof of identity (driver's license, health insurance card, passport, etc.)
▪️ For uniforms, please purchase the official GB uniform (gi and innerwear).
*Once paid, the initiation fee and monthly membership fees are non-refundable, regardless of the reason.
